| As a condition to my accepting to participate I hereby acknowledge that I participate at my own risk. I acknowledge that International Academy of Sport Pty. Ltd.(hereafter known as IAS) and it's servants accepts no responsibility or liability for any injury or loss that I might sustain as a direct or indirect consequence of participating in any activity whether such injury is a consequence of any act or omission by IAS. or it's servants, co-provider, agents, representatives or volunteers, except in regard to any rights I may have arising under the Trade Practices Act 1974. I declare that I am in sound condition and undertake participation with the knowledge of the physical demands required. I consent to receiving any medical treatment, including ambulance transportation that IAS. thinks desirable during or after participating. I acknowledge that the safety precautions undertaken by IAS. are a service to me and other participants, but are not a guarantee of safety. |
| Participants Duties: |
| I agree to follow my instructor's directions at all times. I agree that I will be responsible for any injuries to other persons, damages to the premises, property owned by others, which I may cause by negligent, reckless or irresponsible conduct. All minors must have a parent guardian sign this acknowledgement form to indemnity for minors. By signing my name below, either in person or by one of my representatives, I hereby agree to comply with all of the terms and conditions stated above. |

| By completing this form you are supplying IAS with personal information about yourself. This information is needed to ensure your safety during your time with us. IAS is required to collect this information by our insurance company and the Department of Workplace Health and Safety. The information you provide will not be suppled to any other organisation or used for any other purpose than that which is stated above. |
| Enrolment |
| *Enrolments are subject to the following conditions, which will become legally binding on acceptance of enrolment by IAS. |
| All fees must be paid in full before your course starts. |
| Services |
| *The Company reserves the right to change the particulars of the services, including changes to courses, facilities and dates of programs where circumstances beyond the company's control necessitate such changes or where the number of bookings received do not reach the minimum numbers required to operate a course viably. |
| Other Refund Policy |
| *The following are the IAS Charges and Refunds policies if you cancel or withdraw from a course. o If we reject your application for an IAS course or your visa application is rejected after we have confirmed your place there will be no charge and you will receive a full refund within 30 days. o If you cancel your accommodation 7 days before arrival at your arrival date, you will be charged the accommodation placement fee and a cancellation fee equivalent to 1 weeks accommodation. o If you cancel your accommodation less than 48 hours before arrival you will be charged 2 weeks accommodation fee. o Accommodation fee are monthly refunded providing 2 weeks written notice on that month |
| Cancellation and withdrawal notes |
| o Any enrolment fee, accommodation placement fee and airport transfer fee are non-refundable. o If IAS cancels a course, students are entitled to a refund based on the funds left in the Overseas Students Notified Trust Account. The Trust Funds are maintained as per the regulations of the ESOS Act. o All other courses fee are maintained by company holding account. o If Student wish to withdraw course must notified in writing 1 month prior to end of each term. |
| VISA |
| o Student can study on a Student VISA, Tourist VISA, or Working Holiday VISA. Students should contact their nearest Australian Embassy, Consulate or High Commission for conditions of all VISA categories. |
| Medical Insurance o All Students studying on a Student VISA need to have Overseas Student Health Cover. o IAS are suggesting all students should have Overseas travel Insurance. |
| Resolution of Disputes |
| o If a student wishes to complain about any aspect of the services the complaint should be made in the first instance to the company or course coordinator on site In the event that the matter is not resolved the student or student's agent should make a complaint in writing to the IAS Head Office. o Such complaints will be investigated providing that: o The complaint is received within one month of the termination of the course. o The complaint was initially registered in writing with an on-site coordinator or company Manager. o All invoices relating to the student making the complaint have been settled in full. |
| Liability |
| IAS and its staff and representatives will not be liable for loss, damage ore injury to persons or property however caused, except where liability is expressly imposed by law. IAS will not be liable in the event that any service contracted to be supplied by IAS becomes impossible to supply for any reason or any cause outside the control of IAS. |
| I have carefully read this acknowledgement, conditions and indemnity. I understand it, and voluntarily agree to all of its terms. I understand that these terms apply to me every time I participate at International Academy of Sport Pty. Ltd. activities. |
